Sports Registration

Please use parish code 194SMG-IN when registering.

Go to SPORTS, select REGISTER FOR SPORTS, once you've registered select  VIEW FORMS, please print those forms and send them in with your payment once you have signed them. Next select AMOUNT DUE and print the statement that appears. Please attach your check and the three print outs and send them into the sports ministry office.
